From now on, Ukrainians can sign a declaration with a doctor online.

A patient portal for online submission of declarations to doctors has been launched in Ukraine

The Ministry of Health of Ukraine announced the launch of a patient portal that allows Ukrainians to submit declarations to doctors online. It is available to all adults and teenagers from 14 years old with an electronic signature.

The digital platform “Patient Portal” successfully completed the beta testing stages and is now ready for use. Now, Ukrainians can view and update their personal data in the electronic health system, as well as submit declarations to doctors over the Internet. This will significantly simplify the process that previously required an in-person visit to the hospital.

The patient portal allows not only viewing and changing personal data but also viewing current and previous declarations with doctors, searching for doctors, and submitting declarations online. Teenagers with confirmed civic capacity can also use these functions if they have the necessary documents.

In the future, users will have access to the medical record, including test results, prescriptions, and diagnoses.

To enter the patient portal, one can use one of the verified e-Cabinets developed by both the state system and private patient information systems. The full list of available e-Cabinets can be found via the link on the Ministry of Health’s website.

Earlier, in December 2025, the government approved the implementation plan of the “Health Screening 40+” program, which started on January 30, 2026, aimed at early detection of diseases affecting the quality of life of Ukrainians.
